These changes will affect the interaction between those involved in the pathway of care for stroke, and require new structures, processes and relationships. This session will look at the changes from the perspective of those designing and implementing new commissioning arrangements, and, most importantly, from the perspective of those for whom services are provided.

Prompted by the Stroke Improvement Programme’s call for current issues in improving stroke care, this session will focus on developing services to help stroke survivors once out of hospital. The session will describe and review work on developing effective community services, integrating early supported discharge with community care, and the broader issues of long-term support for stroke survivors including regular reviews of care and working with the third sector.
