Components of a good diagnostic service

Currently, there is no standard COPD diagnosis pathway with a number of different models across the country. Currently, one service model does not fit all locations due to a number of factors including:

  • population demographics
  • staff skill mix
  • resources of both trained, qualified staff and equipment
  • funding arrangements
  • access to full pulmonary function testing

However, it is evident that the following components should be included in all models:

Spirometry is one component of diagnosis and is being undertaken in a number of different service models by different staff with varying expertise. Through the programme of work, different grades of staff have been observed performing spirometry - from Agenda for Change Band 2 to GP Practice Partners and the workstream project teams have expressed that pay is not necessarily significant to the quality of spirometry; it is the skills and competency of the individuals that is important.
