Working with Primary Care – A resource for cardiac networks
Recognising that primary care is the key to achieving whole system change, the Heart Improvement Programme have devised the ‘Working with Primary Care’ resource pack. Designed to assist networks to work more effectively with the primary care sector. The pack provides a range of resources to help improve mutual understanding between primary care and networks and to ensure successful delivery of the CVD agenda.
The resource pack includes information, references, examples of good practice from networks across the country and the results of the 2007 baseline survey of cardiac networks.
