AF case finder
The AF case finder is a tool that was developed by PRIMIS to find patients within a practice that are likely to have AF, but have not been officially diagnosed.
NHS Improvement has not provided any monetary support for this tool, but as it has been used by some areas as well as GRASP-AF the following guides have been created to support those who wish to use it.
