These Children and Young People Cancer Survivorship aftercare pathways have been produced and endorsed as clinical good practice following an extensive consultation process with input from clinical experts and cancers survivors.

These defined pathways demonstrate the evidence emerging from three years of testing and prototyping with clinical teams and cancer patient survivors.

We are really interested in feedback and we will be reviewing comments and making any refinements in March 2012.
