National Cancer Survivorship Initiative: Living with and beyond cancer

Adult Pathway 

Over the past four years work has commenced with patients, NHS clinical teams and third sector organisations to test care and support for those living with and beyond cancer. This work has culminated in the development and pilot testing of pathways of care tailored to individual needs in 4 tumour types (breast,colorectal, lung, prostate) with the aim of improving quality, reducing unnecessary hospital attendances and moving the focus of post treatment care from the disease and treatment to health and wellbeing.

This interactive generic pathway demonstrating examples of care and support from testing work and also signposting to other relevant sources of information. Further updates are planned in June.

Testing pathways of care for those living with and beyond cancer

NHS Improvement is looking for teams to prototype stratified pathways and packages of care for prostate, colorectal and breast cancer patients following initial treatment for cancer. The aim is to test the transferability of stratified pathways and the accompanying packages of care that have been piloted in a number of pilot test sites. This work is a key step prior to national roll out. This testing work is supporting the National Cancer Survivorship initiative (NCSI); a partnership between the Department of Health, NHS Improvement and Macmillan Cancer Support.

 

Evaluation of adult cancer aftercare

A survey of our test sites was carried out by IPSOS-MORI this summer. The survey aims to provide a baseline for the 11 tumour projects within the seven Test Communities, currently testing a new model of care which includes risk stratified care pathways in Adult Cancer Survivorship services. This is part of the National Cancer Survivorship Initiative.
